Documents To Accompany Application

(a) in the case where the applicant will personally provide the services and states that he has a certain standard of education, competence, skill or experience, a certificate showing that the applicant has the standard of education, competence, skill or experience;
(b) in the case of a firm where the partners of a firm will personally provide the services, a certificate showing that the partners of the firm have the standard of education, competence, skill or experience;
(c) in the case of a body corporate or where an individual or the partners in the firm will not personally provide the service, evidence to the satisfaction of the Authority that the body corporate, individual or firm possessing the standard of education, competence, skill or experience;
(d) where premises are required for the purposes of providing the services specified in the application, documentary
(e) in the case of a firm a true copy of registration of the firm and in the case of a corporate body a true copy of the certificate of incorporation and the memorandum and articles of association of the corporate body;

License Condition

To comply with licences (miscellaneous services) regulations 2000.

To provide services as a beautician;

This excludes the following complementary health care services, which a licence is required
Under the licences (complementary health care services) regulations 2004.
(massage therapist, aroma therapist, reflexologist, hypnotherapist, hydro therapist, homeopath, acupuncturist, manipulative therapist, osteopath, energy therapist, color therapist,)

This licence permits you to provide services as a mobile beautician at your clients private accomodation only.

To display this licence at a conspicuous place on the business premises as specified in the licence.

‘’Once Issued, Licence Fee is Non-Refundable’’.

All waste from sinks, wash hand basins and shower should be directed into a grease trap before disposal into the sewerage system

Solid waste collection facilities on site

Qualified and trained personnel on the premises at all times

All practicing personnel should be adequately trained

Good record keeping for clients and product used for treatment

Good practices of personal hygiene by staffs

A clean and hygienic premises at all times

Display in a conspicuous place on the premises the rates of charges in respect of the services on offer.

Must at all times during the duration of this licence, have a valid lease agreement, and copy of same to be provided to this authority. If premise is not being lease then the above is not applicable.
